Declaration of no legal impediment to marriage 

This is a form that I will have filled out and you will sign as close to the actual wedding as feasible. It is usually at the rehearsal. If there is no rehearsal as such I will make every effort to catch up with the day before the wedding.

Under section 50(3) of the Marriage Act 1961, this form shall be printed on the reverse of 1 of the official certificates of marriage.
